The materials for residence permit application

□ 1. 护照及复印件（首页、签证页、最新入境签章页）

□ 2. 体检认证及复印件

2. Physical examination certification and its copy

□ 3. 外国人工作许可证

3. Foreigners’ work permit

□ 4. 外国人临时居住证明

4. Foreigners’ temporary residence

□ 5. 2张2寸白底彩色登记照

5. Two 2x2 color photos

□ 6. 学校公函

6. The school’s official letter

□ 7. 湖北省外专局准予行政许可决定书

7. Hubei provincial Administration of foreign experts affairs’ granting administrative licensing decision

  □ 8. 如有家属须携带关系证明原件和复印件

      8. If there’re family members accompanied, the original and photocopy of the relationship certificate should be provided.

NOTES:

1. 请注意护照、工作许可以及居留许可的有效期限，并在相关证件过期前2-3个月联系学院工作人员，如未及时办理相关证件延期，有可能被遣送，由此产生的责任须自行承担。

Please pay attention to the term of validity of your passport, Foreigners’ Work Permit and Foreigners’ Residence Permit. You shall contact relevant staff in your host department 2-3 months before the date of expiration. Expired passport, Foreigners’ Work Permit and Foreigners’ Residence Permit without in time reapplication will lead to deportation. Applicant is responsible for consequences arising from there.

2. 当有固定住所或更换固定住所以及每次入境后24小时内，请在住所管辖派出所办理境外人员临时住宿登记表。

Please Fill the Registration Form of Temporary Residence for Visitors within 24 hours after upon your every entry in to China at the police station of the residence, putting on record your fixed residence or     the replacement of your permanent residence.
