The materials for the foreigner’s work permit

*所有外文材料都需要翻译件，并加盖翻译公司公章和学院公章

All foreign language materials need to be translated and stamped with the official seals of the translation company and college

*所有材料扫描件为彩色扫描

The scanned copies of all materials are color scanned

材料包括：

The materials include:

□ 1.护照或国际旅行证件原件（信息页，签证页及入境签章页或居留许可信息页）

1.The original passport or international travel document (for the information page, visa page plus the entry signature page, or the residence permit information page only).

□ 2.工作资历证明原件和翻译件原件 (要求：由申请人原工作过的单位出具从事与现聘用岗位相关的、不少于两年的工作经历证明，包括职位、工作时间，或曾经做过的项目，需申请人原工作单位加盖公章或负责人签字，并留有证明联系人有效电话或电子邮件)

2.The original job qualifications certification and original translation. (Requirements: the provided job experience should be issued by the applicant’s original work units, related to currently employed post and no less than 2 years, including position and work period. Should be sealed or signed by head of the unit, and accompanied by contact phone or E-mail address of the prove contact person.)

□ 3.最高学位（学历）证书或相关批准文书、职业资格证明原件和翻译件原件（非中文证书应经我驻外使、领馆或由申请人获得学位（学历）所在国驻华使、领馆或我国学历认证机构认证，且需提供翻译件。港澳台地区亦须公证。最高学位境内获得仅需原件即可。）

3. Applicant’s original highest degree certificate or relevant approval documents or professional qualification certificates and corresponding original translation (Non-Chinese degree certificate should be certified by the Chinese embassies or consulates, and provide Chinese translation.)

□ 4.无犯罪记录证明原件及认证和翻译件原件（应当由申请人国籍国或经常居住地警察、安全、法制等部门出具并经我驻外使、领馆认证或外国驻华使、领馆认证，且需中文翻译件。无犯罪记录签发时间应在六个月内）

4.The original Non-criminal Record and Notarial Certificate of Non-criminal Record and original translation. (should be certificated by the Chinese embassies or consulates, and provide Chinese translation. Valid for six months from the date of issue.)

   □ 5.体检证明认证件（中国检验检疫机构出具的境外人员体格检查记录验证证明或健康检查证明书，签发时间在六个月内）

注意：需要和外国人说明体检地址（武昌洪山区珞狮南路457号）和体检注意事项（申请人需要准备：办理许可通知时已有的体检结果，护照原件，两张登记照以及体检费用200多元）。

5. Certificate of physical examination（the Physical examination verification certificate for overseas personnel issued by China Inspection and quarantine agency or a health check certificate, valid for six months from the date of issue）

Note:

The place for you to get the Certificate of physical examination: Hubei Entry-Exit Inspection and Quarantine Bureau (No 457, South LuoShi Road, Hongshan District, Wuhan)

You need to prepare: your physical examination materials got when you apply the work permit notice, your original passport, two photos, and RMB200 or more.

□ 6.中英文聘用合同或任职证明原件（申请人签名，加盖单位公章，不得涂改，中英文合同需一致）

6.The original Chinese and English employment contract or other employment certification (with applicant’s signature and official seal; not to be altered; the two version should be consistent with each other)

□ 7.若有家属随行，需提供随行家属相关证明材料

包括：①关系证明（例如：妻子/丈夫结婚证明，子女出生证明）

②护照信息页

③体检证明（18周岁以上家属））

  备注：妻子/丈夫结婚证明和子女出生证明需要公证书以及翻译件

7. If accompanied by family members, you need provide their related certification materials, which include:

① Relationship certificate(e.g. wife/husband marriage certification, son/daughter birth certification)

② The information page of the passport

③ The physical examination of adults

Note：Either wife/husband marriage certification or son/daughter birth certification requires notarizations and translations.
